Condition

Flattened Cheeks

Patients often describe their face as looking flatter or more tired without being able to say why. Frequently the change is in the mid-face, where the cheek used to provide light, projection and support for everything below it.

What is Flattened Cheeks?

The cheek is supported by deep fat compartments, bone and ligaments. With age these compartments reduce and descend, and the underlying bone remodels. The result is a flatter mid-face, a less defined transition to the lower lid, and heaviness lower down because the tissue is no longer supported from above.

Common signs and concerns

  • Less light reflecting from the front of the cheek
  • A flatter profile when viewed from the side
  • Deepening of the fold between nose and mouth
  • Under-eye hollowing appearing at the same time
  • Skin looking looser along the jaw as mid-face support reduces

Possible contributing factors

  • Age-related fat and bone changes
  • Significant or repeated weight loss
  • Genetic facial structure
  • Sun exposure and smoking
  • High-intensity exercise with low body fat

When to seek professional assessment

This is an aesthetic concern rather than a medical one. Assessment is worthwhile if you are considering treatment, so that the plan addresses the actual cause rather than adding volume where it does not belong. Rapid, unexplained facial fat loss should be reviewed by your GP.

How Dr Farzana assesses this concern

Assessment is anatomical rather than product-led. Dr Farzana looks at facial proportion, skin quality, volume distribution and movement, then discusses what is contributing to the appearance you have noticed. Options are explained with their limits, alternatives and recovery, and nothing is treated on the day unless you are comfortable and suitable.

Treatments that may be considered

These are options offered by Dr Farzana that may be discussed for this concern. Which, if any, is appropriate for you is decided only after a private medical assessment.

Suitability and important information

This page is general information only and is not medical advice or a diagnosis. Suitability for any treatment is confirmed at a private medical consultation, including a review of your history, medication and goals. Risks, alternatives, aftercare and realistic outcomes are discussed openly. Results vary between individuals and no treatment is suitable for everyone.

Frequently asked questions

Why do cheeks flatten with age?+

The cheek is supported by deep fat compartments, bone and ligaments. With age those compartments reduce and descend and the underlying bone remodels, so the mid-face looks flatter and provides less support to everything below it.

Why does my whole face look tired when only the cheek has changed?+

Mid-face support affects the areas around it. As projection reduces, under-eye hollowing and heaviness along the jaw often appear at the same time, which is why the face as a whole can look more tired.

Will my face look overfilled?+

Dr Farzana works conservatively and in stages, prioritising proportion over volume. Treating a little, reviewing, then deciding together whether anything further is needed is the standard approach.

Can collagen stimulation be used instead of filler?+

For some patients it suits them better. Collagen-stimulating options work gradually and appeal to those who prefer a slower, structural change, and suitability is decided during your consultation.

How long might results last?+

Longevity depends on the option chosen, the area treated and individual factors such as metabolism and weight change. Expected duration and any maintenance are explained before you consent.

Can weight loss or exercise contribute?+

Significant or repeated weight loss and very low body fat both reduce facial fat, so mid-face flattening is common in that setting. It is worth mentioning at consultation because it affects planning and longevity.

When should this be reviewed by a GP?+

Rapid, unexplained facial fat loss should be reviewed medically rather than treated cosmetically. Otherwise, this is an aesthetic concern and assessment is entirely elective.
